Service Agreements

An NDIS service agreement is a formal arrangement or contract between a participant in the National Disability Insurance Scheme (NDIS) and a service provider. This agreement outlines the terms and conditions of the support and services that the provider will deliver to the participant.

It serves as a mutual understanding between the participant and the service provider, ensuring transparency and clarity regarding the support arrangements. A Service Agreement includes participant details, description of services that will be provided, costs and funding, cancellation and changes, complaint and dispute resolution, start dates and signatures.
